The largest size offered here is 1280 x 720 (HD). The tool does not produce 4K thumbnails, and it does not upscale: you get the file exactly as YouTube stores it.
Why the 1280 x 720 version is sometimes missing
maxresdefault does not exist for every video. It is typically absent on older uploads, on videos uploaded in low resolution, and on videos without a high-resolution thumbnail. YouTube then answers with a small grey 120 x 90 placeholder instead of the picture. The tool detects that, marks the Maximum Resolution card as "Not available for this video" and disables its download button. Use High Quality (hqdefault) instead, and crop the black bars at the top and bottom if you need a clean 16:9 image.

Shorts use the same video IDs and the same thumbnail file names as regular videos, so a youtube.com/shorts/... link works here without any extra step.
One thing to expect: Shorts are vertical (9:16), but these thumbnail files keep the landscape dimensions listed above. The image you download is therefore usually a cropped or padded version of the vertical frame, not a full-height 9:16 picture. For a vertical cover image you will need to recreate it from your own source footage.
